Anyone who wishes to register with the Practice should ask at Reception and complete a Registration form and Patient Questionnaire. We ask that you please read and complete the form carefully and correctly, as if this is incorrectly filled in this will delay your registration.
The form requires your NHS number, you can get this on a repeat prescription a hospital letter or from your previous GP Surgery. Please also ensure that you sign the bottom of the form.
